APPLICATION: An extensive application is completed by each nurse. Including appropriate license verification, past educational and work history, drug free work place information, criminal background check and previous employment references.

TESTING: All new hires will be tested in their area of expertise. A score of 80% or better is generally required. We do have some clients that require a score of 85% or better. Your recruiter will help to determine which score will be required at your time of hire.

ANNUAL UPDATE: On every anniversary of the date of hire for each employee at Emerald City Medical Staffing, the employee is required to come into the office and complete an annual update screening. This includes all JCAHO/OSHA and client requirements.

HEALTH RECORDS REQUIREMENTS:
- Current PPD/TB skin test and previous test to show 2 PPD’s in the past 13 months, if you are a positive PPD then you must provide us with a negative Chest X-ray (additional requirements for +PPD maybe requested depending on the clientele you are being submitted too), an annual TB questionnaire will also be required
- Proof of 2 MMR vaccines if you were born after 1957 or positive Titers for all 3 diseases (Measles, Mumps and Rubella). If you were born before 1957, then you must have had 1 MMR vaccine or positive Titers for all 3 diseases.
- Hep B series completion or positive Titer (there are some clients that require you have completed the series, had a titer drawn and if you are still negative for antibodies, then you will fill out a declination waiver)
- Proof of 2 Varicella (chicken pox) vaccines or a positive Titer
- Proof of a physical exam, stating that you have no restrictions or limitation. This should be at a minimum the equivalence to a comprehensive health history statement.

LICENSE VERIFICATION: All professional licenses will be verified by the State of Washington using their website at:
https://fortress.wa.gov/doh/providercredentialsearch/
We will print off the results and keep it in your personnel record. This is also done at your annual update every year.

SKILLS CHECK LIST: Each applicant must complete a comprehensive skills inventory that is kept in the personnel file in the office and is updated annually. If the nurse works in a specialized area this may not be necessary or a skill list for their area will be provided.

REFERENCES: At least two references verifying work experience in clinical areas are required for employment. No Emerald City Medical Staffing nurse will be placed without references. Telephone references, when available, are allowed and acceptable.

CERTIFICATIONS: CPR or BLS must be from an approved American Heart Association course and must be for Health Care Providers. Your card must have the American Heart logo and say specifically that you are a “Health Care Provider”. ACLS/PALS/NRP must also say “Provider” on the front of the card.

EVALUATION: You must bring with you on your appointment day, a performance evaluation completed by a previous employer. If you need a blank document in order to obtain this, please call the office and we will provide this for you.

EMPLOYEES FILE: A complete and current file is kept on all employees; including application, license and BLS certification, test results, references, medical history and any continuing education courses completed. An employee’s file also contains appropriate documentation relative to the applicant’s identity and work authorization as required by INS rules.

INTERVIEWING: All prospective employees are interviewed by the office personnel. During the interview, emphasis is placed upon work history, clinical expertise and review of the references. At this time, information and performance requirements are given to the applicant regarding Emerald City Medical Staffing's policies and procedures.

PLACEMENT AND ORIENTATION: Assignment of all medical professionals are made by our office personnel according to the skill and expertise of the professional and the needs of the client. Emphasis is placed on continuity of service whenever possible. Orientation policies and procedures of all clients and facilities are adhered to by Emerald City Medical Staffing.

CONTINUING EDUCATION: Emerald City Medical Staffing, Inc. currently does not pay for any continuing education or renewal of any certifications. We do reserve the right to change and/or modify this policy at any time.
